Basic Occupational Therapy Evaluation
Service Description
A current OT treatment plan, progress report, or evaluation (completed within the past 6 month) is required prior to the start of therapy services, including therapy camp. A Basic OT Evaluation will explore your child’s unique developmental profile & strengths, their daily life contexts, and factors that may be contributing to challenges they are experiencing. A Basic OT evaluation includes: -Completion and review of intake paperwork -30 minute Parent Interview -45 minute in-person evaluation, including therapist interaction with your child and clinical observations. Standardized assessment may be used -Scoring of online questionnaires -Family/therapist collaboration to determine goals -Development of a brief summary report and a plan of care specific to the services provided at Wildcraeft Works prior to the start of services
